Why is “what’s your wound” said in every flashback?

It's a call-back (this episode was all about the call-backs) to the first thing Morgan said to him in episode one.

On a deeper level, I think it was Rick asking himself what his emotional wound was.

I felt like the voices changed when it was being said. Feel like it was voices of characters we knew but need to rewatch

Also an interesting call back to the phone calls at the prison, where he also heard the voices of people no longer around.

I heard Morgan & Abe but I missed Lori, even on the second go-around.

Since Rick saw Shane, Hershel and Sasha, I think it suggests that Rick felt most 'guilty' about these 3 persons, among others.
He apologized to Shane for what he did to him, to Hershel about what happened to Beth, Glenn and Maggie (apparently for his inability to protect them well and, sort of, failing Hershel entrusting them to his care...and might be trying harder would make a difference...but Hershel told him that his girls are strong) and to Sasha.

As far as I can remember, he didn't. Though I felt like Sasha was there for a different reason, anyway.

At first I thought it was to represent the optimism that Bob and Tyreese had (and maybe that's what it was to an extent), but then I remembered how she died.
I believe that's why he needed to see her, to hear what she had to say and convince himself that he could do it.

yeah, on the bonus scenes on TTD Gimple and Yvette Nicole Brown confirmed it was Beth right before Hershel showed up. They said the voice was all connected to that particular person/scene. What's really funny, is I rewatched with captions, and they listed Morgan, Abe, and Lori, but for Beth all it identified it as was 'woman's voice':p
